Social Support Program

Social Support is a Home and Community Care (HACC) Program developed to prevent isolation by providing social links for older people, people with disabilities and carers.

It provides regular contact, support and security for older people and people with disabilities who are housebound or isolated due to frailty, lack of confidence, geographic or other factors.

FRIENDLY VISITING SERVICE

This is a service that establishes friendships between older people or people with a disability, and volunteer visitors.

Formerly known as Do Care, the Friendly Visitor service matches a volunteer visitor with an older person with a view to establishing a mutual friendship.

Each volunteer visitor, calls on his/her friend at home.  Friends can share in many activities; most often they just sit and talk but sometimes they may decide to go out.  The friends, between them, choose the most suitable times to get together.  The volunteer visitor does not interfere in their friends life, they simple offer friendship.

Friendly Visitors are people from the community who have an empathy with , and enjoy the company of older people.  Anyone with the time and commitment can become a Friendly Visitor.

Wimmera Volunteers offers support and training, and matches you to someone you have a common interest with.

VOLUNTEER TRANSPORT PROGRAM

The Volunteer Transport Program was developed in recognition of the difficulties some frail, elderly and disabled people have in accessing appropriate transport.

The Volunteer Transport Program consists of a network of vehicles and drivers available throughout the Wimmera region to provide transport assistance in specific circumstances.

The Home And Community Care Program (HACC) is a range of services provided to assist in the prevention of premature or inappropriate admission to long term Residential Care.

The services are aimed at contributing to the social and emotional wellbeing, security and independence of frail older people, people with disabilities and carers.

If your are eligible for the service, a registered volunteer driver will be requested to provide you with transport to the destination required.

Assistance is subject to meeting the eligibility criteria and the availability of a volunteer driver.

Wimmera Volunteers provides support and training to our volunteer drivers.
