Wimmera Volunteers administers the funding for the Hospital to Home Program (H2H).  H2H is a Victorian State Government initiative that was developed to support people aged 65 years and over with the provision of services in the home following discharge from hospital.  Services are provided on a short-term basis to assist patient recovery.
